Islet Cell Transplantation for the Treatment of Diabetes

In islet cell transplantation, insulin-producing beta cells are taken from a donor's pancreas and transferred into a person with diabetes. Once transplanted, the donor islets begin to make and release insulin, actively regulating the level of sugar in the blood.

Benefits of Islet Cell Transplantation

A successful islet cell transplant can significantly improve the quality of life for a person with diabetes.
Once transplanted, the islet cells automatically monitor sugar in the body and can regulate insulin levels by delivering the appropriate amounts needed to normalize blood sugar levels, even as the body's needs change (for example, after exercising or eating).
Successful islet cell transplantation can provide the following benefits:

  • It can eliminate the need for frequent blood sugar measurements and the need for daily insulin injections. Although only a few people are completely free of insulin injections a year after transplantation.
  • It can provide more flexibility with meal planning.
  • It can help protect against the serious long-term complications of diabetes, including heart disease, kidney disease, stroke, and nerve and eye damage.

Risks of Islet Cell Transplantation

As with any organ or tissue transplant, rejection of the donor cells is the greatest challenge in those with diabetes. The immune system serves to protect the body from "invading" substances that do not belong -- bacteria and viruses, for example. Even when needed donor tissue is transplanted, the recipient's immune system recognizes it as "foreign" and tries to destroy it. This attack on the donor tissue is called "rejection."
All transplant recipients must take, for the rest of their life, strong drugs to suppress the immune response and prevent rejection. Many of these drugs have serious side effects. The long-term effects of these immunosuppressive or anti-rejection drugs are not yet known, but it is suspected that they may increase the risk of cancer.
Scientists developed the procedure for transplanting islet cells to treat diabetes in the 1960's. The first transplantation attempts, which began in the 1990's, succeeded only 8% of the time, which was attributed to the fact that anti-rejection drugs available at the time interfered with insulin's effectiveness.
But in 1999, a clinical trial conducted at the University of Alberta in Edmonton, Canada, brought new hope. Using enhanced techniques to collect and prepare the extremely fragile donor islet cells, as well as using improved anti-rejection drugs, the researchers achieved a 100% success rate. All of the patients in their trial were freed from the need for insulin for at least one month.

Oral Diabetes Medications

Oral diabetes medications -- diabetes pills -- help control blood sugar levels in people whose bodies still produce some insulin (the majority of people with type 2 diabetes). These diabetes drugs are usually prescribed to people with type2 diabetes along with recommendations for making specific dietary changes and getting regular exercise. Several of these drugs are often used in combination to achieve optimal blood sugar control.
Remember that people with type 2 diabetes tend to have two problems that lead to increased sugar (glucose) in the bloodstream:

  • They don't make enough insulin to move glucose into cells where it belongs.
  • The body's cells become "resistant" to insulin (insulin resistance), meaning they don't take in glucose as well as they should.

In time, people with type 2 diabetes develop what's called "beta-cell failure." This means that the cells in the pancreas that make insulin no longer are able to release insulin in response to high blood sugar levels. Therefore, these people often require insulin injections, either in combination with their diabetes pills, or just insulin alone to manage their diabetes.

Pancreas Transplantation

Ultimately, the goal in the management of type 1 diabetes is to provide insulin therapy in a manner that mimics the natural pancreas. Perhaps the closest therapy available at this time is a transplant of the pancreas. Several approaches to pancreatic transplantation are currently being studied, including the whole pancreas and isolated islet cells (these groups of cells contain beta cells that are responsible for insulin production). Data available from 1995 indicates that almost 8,000 patients underwent pancreatic transplantation. Most patients undergo pancreatic transplantation at the time of kidney transplantation for diabetic kidney disease.
Transplantation is not without risk. Both the surgery itself and the immunosuppression that must occur afterward pose significant risks to the patient. For these reasons, the kidney and pancreas are usually transplanted at the same time. At present, there is disagreement about whole pancreas transplantation in patient’s not currently requiring kidney transplantation. The issue of whether the benefits outweigh the risks in these patients is under debate. There is also a chance that diabetes will occur in the transplanted pancreas. Selectively transplanting islet cells is an interesting alternative to whole pancreas transplantation. However, the concern over rejection remains. Attempts to disguise the islet cells in tissues that the body won't reject (for example, by surrounding the islet cells by the patient's own cells and then implanting them) are underway. In addition, researchers are exploring artificial barriers that can surround the islet cells, provide protection against rejection, and still allow insulin to enter the bloodstream.
