Gout is a metabolic disorder that occurs due to the presence of highly raised serum uric acid levels. It is a slowly progressive disease which mimics the symptoms of osteoarthritis.
It usually affects people on a high protein diet or which are ignorantly consuming proteins in excess of the body's requirements. Non – vegetarians are found to be more prone to develop this disease.
It is one of the most common types of inflammatory joint conditions and is very closely linked to the development of cardiovascular problems as well. Its incidence is almost twice of that of the white men in the age group of 26 to 34 years.
Persons taking a high protein diet with low physical activity and especially males in the age group of 25 years and above are said to be at an increased risk of the development of gout.
Raised serum uric acid levels are directly linked to the development of gout in any individual. Obesity, kidney problems and other metabolic problems only add to the severity of the disease.
CHARECTRERISTIC FEATURES OF THE DISEASE
Gout is usually a chronic episodic condition in which the flare ups of the disease occur intermittently. The time period of the recurrence of flare ups is very uncertain.
Every flare up condition may persist for days causing pain in one or more joints of the feet or hands and then slowly remit with or without medications.
Usually, the smaller joints of the hands and feet are involved initially. Later on, the bigger ones come into picture. This is the normal course of the disease and the time duration of this may vary from person to person.
STAGES OF GOUT
Stage – 1
In this stage, there are no overt symptoms and the patient is suffering from the disease but without knowing it. Deposition of urate crystals and tissue damage as a result of it has already started.
Stage – 2
This is the stage of flare ups where inflammation of the joints commonly of the lower extremities occurs. In this case, metatarsopharyngeal joints of the big toe is usually inflamed in 50% of the patients.
This stage may last from a few days to weeks. It is usually accompanied by mild to severe pain in the toe joint.
Stage – 3
This is the intermediate period between to flare ups where the serum uric acid levels are constantly high but the symptoms of the previous flare up have subsided. The duration of this stage progressively shortens with the chronicity of the disease.
Stage – 4
This is the stage of chronic gout where the disease has completely caught hold of the patient's body. It includes arthritis, inflammation, redness and swelling of one or more joints involved.
Patients in this stage of the disease may also have problems like the formation of tophi (soft crystal deposits of uric acid in various body parts like elbows, ears, distal finger joints etc.
