Gout is a disease of the joints, characterised by sudden attacks of severe joint inflammation, in which they become swollen, red and very painful. The cause does not lie in the joints, but in the blood. It is due to a raised blood level of a substance called uric acid. Uric acid is a natural substance that results from the breakdown of food. This waste product is excreted in the urine. Certain conditions can arise that may lead to the accumulation of uric acid in the body. As the level rises, uric acid finds its way into joints and crystallises there. The crystalline form of uric acid can trigger gout attacks. Uric acid can also accumulate in other parts of the body such as the earlobes, elbows or ankles.

A gout attack often starts explosively, said Dr Leong Khai Pang, a senior consultant with the rheumatology, allergy and immunology department at Tan Tock Seng Hospital. Some patients may experience tingling at the site just before an attack. Even though any joint can be affected, the base of the big toe is involved most of the time. Usually, a single joint is involved in an attack. The pain can be so severe that the patient cannot use the limb. Even a light touch (such as the weight of a blanket) can provoke unbearable pain. The whole attack can last up to a week or more. Some patients develop fever or malaise during an attack as well. The attacks can recur on an irregular basis. People who develop gout are usually males above 30 years old and females above 60 years. Those who are obese or suffer from kidney disease may also develop gout. Gout is due to the build-up of uric acid in the body. The reasons for excessive production of uric acid include genetics, overindulgence in certain foods (such as innards, broccoli or tuna), and the use of certain medications. There is no simple way to prevent a person from developing gout because it is a result of genetics, lifestyle (food, alcohol intake, activity, water intake) and medication use. However, it must be treated once it develops. Improvement in one's lifestyle is then mandatory. These changes are meant to reduce the level of uric acid in the blood. These include avoiding alcohol, drinking enough water, reducing obesity and avoiding foods that are high in purine (which is converted to uric acid in our bodies). If lifestyle modification is not enough, drugs to reduce the level of uric acid have to be prescribed.
