Gout is a disease of the joints, characterised by sudden attacks of severe joint inflammation, in which they become swollen, red and very painful. The cause does not lie in the joints, but in the blood. It is due to a raised blood level of a substance called uric acid. Uric acid is a natural substance that results from the breakdown of food. This waste product is excreted in the urine. Certain conditions can arise that may lead to the accumulation of uric acid in the body. As the level rises, uric acid finds its way into joints and crystallises there. The crystalline form of uric acid can trigger gout attacks. Uric acid can also accumulate in other parts of the body such as the earlobes, elbows or ankles.
Homeopathy focuses on finding the cause of the inner imbalance in the body that predisposes certain individuals to be more susceptible to developing gout, said Dr Lilly Leong, a homeopath with Back II Life Chiropractic and Homeopathic Clinic. Homeopaths need to find out why the body has difficulty in getting rid of uric acid. Thus, a constitutional approach in prescribing a homeopathic remedy is strongly recommended. However, there are a few remedies that can help with alleviating symptoms of gout, such as aconite for any sudden onset of pain with anxiety, belladonna when the joint is very hot with intensive throbbing pain, colchicum for gout in the big toe or heel and ledum for extremely swollen joints with mottled, purplish skin tone. Alternating application of hot (three minutes) and cold (30 seconds) compresses helps to ease the pain. A change of diet is a must to keep gout at bay. Decrease your meat intake and avoid all forms of alcohol especially beer. Increase water intake as this helps with "flushing out" excess uric acid.
Ayurveda and Gout
A variety of herbs are used in Ayurveda to alleviate gout, said Dr Vikram Chauhan. These are often applied externally or ingested. Some of these herbs include the nirgundi, prasarini, aloe juice, guggul and the Indian ginseng (ashwagandha). Healing gout includes decreasing the production of uric acid and increasing its excretion in the urine.A vegetarian diet is recommended. Celery seed, for instance, helps to dissolve urates, while dandelion leaves helps to eliminate them in the urine.
A dose of 30 to 40 ml of a tincture of celery seed and 30 to 40 ml of dandelion leaf extract can be taken weekly, and divided into doses to be taken three times daily. A herbal tea of the following herbal combination called navkarshik churna can be taken three times a day. The combination includes triphala which removes toxins from the body, and neem, katuki and manjistha — all of which help to purify the blood and correct the metabolism of pitta and heat regulation in our body. These herbs act together and are effective in swollen, painful joints.
Modern medicine and gout
A gout attack often starts explosively, said Dr Leong Khai Pang, a senior consultant with the rheumatology, allergy and immunology department at Tan Tock Seng Hospital. Some patients may experience tingling at the site just before an attack. Even though any joint can be affected, the base of the big toe is involved most of the time. Usually, a single joint is involved in an attack. The pain can be so severe that the patient cannot use the limb. Even a light touch (such as the weight of a blanket) can provoke unbearable pain. The whole attack can last up to a week or more. Some patients develop fever or malaise during an attack as well. The attacks can recur on an irregular basis. People who develop gout are usually males above 30 years old and females above 60 years. Those who are obese or suffer from kidney disease may also develop gout. Gout is due to the build-up of uric acid in the body. The reasons for excessive production of uric acid include genetics, overindulgence in certain foods (such as innards, broccoli or tuna), and the use of certain medications. There is no simple way to prevent a person from developing gout because it is a result of genetics, lifestyle (food, alcohol intake, activity, water intake) and medication use. However, it must be treated once it develops. Improvement in one's lifestyle is then mandatory. These changes are meant to reduce the level of uric acid in the blood. These include avoiding alcohol, drinking enough water, reducing obesity and avoiding foods that are high in purine (which is converted to uric acid in our bodies). If lifestyle modification is not enough, drugs to reduce the level of uric acid have to be prescribed. Occasionally, joints that have been severely damaged by gout need to be replaced with an artificial one, if available. Only a few types of joints can be replaced, for example, the knee.
